Visit https://app.truemed.com/fitness/qualify/tm_qual_b0e4byvimm and complete health assessment. A Truemed licensed provider will review your answers to determine eligibility. If approved, you will receive a Letter of Medical Necessity (LMN).
Truemed charges $30 for the health assessment. If you do not qualify, you will be reimbursed.
Complete Membership Purchase in StreamFit
Use your regular purchase method (credit or debit) in StreamFit. (Some HSA cards may work directly in StreamFit, but not all. This is dependent on your carrier.)
Submit for Reimbursement
Follow the guide included with your LMN to submit for reimbursement. You will submit your recepits and LMN to your carrier. Claims are usually paid out in 2-4 weeks. Truemed guarantees reimbursement once qualified!

No, PHYT CLE uses Truemed to qualify for gym memberships and personal training. You do NOT need to qualify for HSA/FSA use for PHYT for Function's Physical and Occupational Therapy services. Our licensed therapists are direct-access, meaning you do not need a LMN or a prescription from your physician to make an appointment to use HSA/FSA.
